Gynecomastia is enlargement or swelling of the breast tissue in men, affecting one or both sides. It is a common condition — many men carry excess breast tissue across the chest or have “puffier” nipples — and it can be genetic or the result of an estrogen and testosterone imbalance following puberty. Because it cannot be corrected with exercise, surgery is the reliable solution. Male breast reduction reduces excess breast tissue, reduces the swollen appearance of the nipples, and restores a flatter, more masculine chest shape.
Surgery is performed under anesthesia at our Brentwood, TN surgical center. Which technique your surgeon uses depends on what your chest tissue is actually made of, and the two are often combined.
When the enlargement is primarily excess fatty tissue — sometimes called pseudogynecomastia — liposuction is used. A thin cannula is inserted through small incisions and the fat is removed by medical suction.
When the enlargement is primarily excess glandular breast tissue, that tissue is removed surgically through incisions in the chest. Glandular tissue does not respond to liposuction, so excision is the only way to address it.
Excess skin is often removed from the chest at the same time, and the areolae may be resized for better proportion once the underlying volume is gone. Incisions are then closed with sutures.
An elastic compression garment is worn for a period after surgery to control swelling and help the skin conform to the new chest contour. Bruising is expected and fades with time. Most patients take time off work until their surgeon clears them to return, and avoid heavy lifting and sports for several weeks.
Gynecomastia surgery is performed under anesthesia and carries the usual surgical risks of infection, bleeding, and reaction to anesthesia. Temporary or permanent changes in nipple and chest sensation are possible. Asymmetry, contour irregularities, or loose skin can occur and occasionally require a revision. Incisions leave permanent scars, though they are placed to be as inconspicuous as possible. Smoking is a significant concern with this procedure: it impairs healing, raises the risk of complications, and reduces skin elasticity, which can leave the skin drooping. If the underlying cause is hormonal or medication-related, your surgeon may want that evaluated before surgery, since an untreated cause can allow tissue to return.

Breast augmentation and breast lift surgery can both improve breast shape, but they address different concerns. Augmentation adds volume, while a lift raises and reshapes breast tissue that has begun to sag. Some patients benefit from combining the two.
